Full-Time Case Manager – Services to Trauma Affected Refugees (STAR)

The full-time (40 hours/week) STAR Case Manager, reporting to the STAR Project Coordinator, will provide trauma-informed case management to refugee individuals and families whose experiences with trauma impact their ability to adapt and thrive in their new environment. Services are provided in a variety of settings, either virtually, in-office or in clients’ homes. Services are provided in the language most comfortable for each client, either by staff members who speak the same language or by collaborating with trusted professional interpreters as needed. Case Manager will support families enrolled in refugee mental health programming funded through the Office of Refugee Resettlement.

STAR Case Manager Job Duties:

  • Assist with screening and enrollment of project clients.
  • Co-develop family and individual wellness plans with clients that detail treatment goals and benchmarks.
  • Collaborate closely with STAR Project mental health therapists to holistically support clients with identified case management needs.
  • Complete necessary benefit applications and obtain supports, services, and resources for clients.
  • Empower clients by helping them learn about available resources and how to navigate systems in the U.S. and the St. Louis region.
  • Utilize organization’s Language Services department to work sensitively and flexibly with interpreters if unable to communicate directly with client in same language.
  • Attend regular Case Manager meetings and STAR team meetings to review wellness and discharge plans, as well as progress toward achievement of client goals.
  • Approach all clients with cultural humility.
  • Approach client services from a trauma-informed perspective, with support and professional development opportunities from Monarch.
  • Document all project activities and client interactions in the agency Electronic Health Records system in an accurate and timely manner.
  • Other duties as assigned.

Minimum Requirements:

  • Direct case management or human services experience
  • Valid driver’s license and access to reliable personal transportation
  • Successful completion of state and federal background checks, Missouri Family Care Safety Registry check, and employability check
  • Ability and desire to work in a highly diverse team of people who are deeply committed to serving our clients and who show up fully-engaged to creatively meet clients’ and colleagues’ needs.
  • Ability and willingness to meet with clients in their homes and community locations
  • Microsoft Office suite experience
  • Bilingual language skills and/or familiarity with working with interpreters is a plus.
  • Lived experience, prior work with refugee clients, work experience in refugee camps abroad, or with migration-related trauma is preferred.
